The Clinical Award Application and the new Drive to Zero Harm Leadership Award Application will be open through July 10, 2019

SCHA is pleased to announce that the 2019 Clinical Award Application is now open, as well as the newly-launched Drive to Zero Harm Leadership Award. Both applications can be accessed here.

This is a great opportunity for hospitals to showcase clinical excellence as well as exceptional leadership. Hospitals may apply for one or both awards through July 10, 2019.

Clinical Award Categories Include: 

CLABSI (ICU or special non-ICU Hematology/Oncology or Renal Dialysis Units)

SSI knee replacement 

SSI hip replacement

SSI colon surgery

SSI abdominal hysterectomy September 2012 - December 2018:  12, 18, 24, 30, 36, 42, 48, 54, 60, 66, 72 or 76 consecutive months 

Hospital onset C. diff*January 2015 - December 2018:  12, 18, 24, 30, 36, 42 or 48 consecutive months 

Hospital onset MRSA* January 2013 - December 2018:  12, 18, 24, 30, 36, 42, 48, 54, 60, 66 or 72 consecutive months

 (*Hospital owned Rehab Hospitals now qualify)

The Drive to Zero Harm Leadership Award:

Leaders are often a step or two away from the day-to-day clinical work where so much harm is prevented, but establishing and fostering a culture of safety starts with them. The Drive to Zero Harm Leadership Award recognizes and celebrates leaders who strive for Zero Harm at every level of their organization. Leaders nominated for this award should have a demonstrated track record of commitment of creating sustainable cultures of safety throughout their healthcare system.
