Seeking Nominations for 2014 Lewis W. Blackman Patient Safety Awards

Do you know someone who has made a significant impact on patient safety for SC patients?

Do you know or work with someone who has made a significant impact on patient safety for South Carolina patients? Nominate them for a  2014 Lewis W. Blackman Patient Safety Award, which was established in 2008 by the founding partners of the South Carolina Patient Safety Symposium: Health Sciences South Carolina (HSSC); PHT Services, Ltd. (PHTS); the South Carolina Hospital Association (SCHA); and Mothers Against Medical Error (MAME). It is a dynamic way to celebrate the achievements of individual patient safety champions and to establish a benchmark of excellence in health care quality and patient safety for our state.

Nominations will be accepted and awards will be given in the following four categories:

1. The Caregiver Award

Direct caregivers or frontline staff whose passion for quality improvement and patient safety has resulted in the successful implementation of unit or system-wide changes that facilitate safer care at the bedside

2. The Health Care Executive Award

Health care executives or leaders who are not direct caregivers, but whose vision, guidance and support have played a critical role in quality improvement and patient safety

3. The Patient Advocate Award

Advocates not involved in direct patient care who have actively demonstrated exemplary dedication to quality improvement and patient safety

4. The Innovation and Research Award

Innovators or researchers involved in quality improvement and patient safety research or the development of new technologies to improve patient outcomes

5. The Student Award

A $500 scholarship and plaque will be awarded to a student who demonstrates notable leadership, commitment and/or innovation in the areas of quality improvement and patient safety
