NQF Approves Updated List of Serious Events

The National Quality Forum, which endorses voluntary consensus standards, recently approved an updated list of Serious Reportable Events. The list of largely preventable errors and events includes four new SREs: death or serious injury of a neonate associated with labor or delivery in a low-risk pregnancy; patient death or serious injury resulting from the irretrievable loss of an irreplaceable biological specimen; patient death or serious injury resulting from failure to follow up or communicate laboratory, pathology or radiology test results; and death or serious injury of a patient or staff member associated with the introduction of a metallic object into the MRI area.

In addition to hospital care, the updated list applies to office-based practices, ambulatory surgery centers and skilled nursing facilities. More than half of the nation’s states use the list in public reporting programs.
