Zero Harm Awards


 

Do no harm.

The phrase is at the core of the Hippocratic Oath, the commitment made by physicians and healthcare workers across the world as they embark on their careers practicing medicine. Put it another way: zero harm is the goal.

The precept has been around since Ancient Greece, and each epoch of medicine has practiced a version of it. In our modern healthcare world though, accomplishing zero harm has become a much more complicated enterprise. “Medicine has become the art of managing extreme complexity — and a test of whether such complexity can, in fact, be humanly mastered,” writes Atul Gawande, MD, in his critically acclaimed book The Checklist Manifesto. Gawande, a surgeon by training, sees this struggle daily in the operating room, where a team of people have to act in precise concert with one another without missing a dizzying array of crucial steps. Thus, he argues, healthcare providers need to focus not just on expertise and knowledge, but also on delivering highly reliable care that mitigates against the realities of increasingly complex systems.

Over the last few years, South Carolina hospitals have fully embraced this new approach towards improving quality of care and are now a national model for how to implement high reliability programs. It all starts with our SC Safe Care Commitment, a collaboration with our partners at The Duke Endowment and The Joint Commission Center for Transforming Health that has championed a statewide effort at creating a culture of high reliability and reduce harm in our hospitals. Borrowing from the approaches in the aviation and construction industries that Gawande describes in his book, hospitals commit to changing their culture to deliver exceptionally safe and consistently high quality results over long periods of time in complex, high-risk environments.

Since 2013, we’ve recognized these efforts through our Certified Zero Harm Awards program. The goal of this program is to publicly recognize the progress South Carolina hospitals have made in targeting and eliminating areas of harm, like bloodstream and post-surgery infections through the kind of system and procedural improvement fostered by a culture of learning and accountability. Thanks to those improvements, hospital units across the state are boasting 12, 18, 24 and even as much as 45 months of harm-free stretches, successes which are validated by South Carolina’s Department of Health and Environmental Control.

After four years of dedicated effort, 43 hospitals—two-thirds of our members—have now won at least one Zero Harm Award. We presented roughly 75 awards each of the first two years. In the third year, the number of award winners jumped by almost 50%, to a total of 110. In 2017, that number climbed to 119. That makes for 377 Zero Harm Awards in the first four years of the program.

This year’s award winners alone amassed some impressive numbers:

  • 62,344 central line days without an infection,
  • 9,741 harm-free surgical procedures,
  • and 17 hospitals had 45 consecutive months without a specific type of harm event.

 

“This early success—expressed in lives saved and the resulting economic impact—strengthens our resolve to focus on safety, raise the bar of expectations, and challenge our hospitals to pursue zero harm,” says SCHA President and CEO Thornton Kirby.
