Richland Care

Richland Care (RC) is a partnership of community organizations that want to make health care accessible to those that do not have health insurance and cannot afford care. It serves as a medical home network providing access to primary care, pharmaceuticals, specialty care, and hospital care, behavioral health, disease management, and education for low-income, uninsured Richland County residents. Created in 2001, Richland Care services also include care management activities and supportive services for participants with hypertension and/or diabetes and demand management capability for participants who utilize the emergency departments. Health education and wellness/health promotion services are available to all participants. These activities are supported by the Healthwise Handbook (a self-care reference guide), newsletters and a 24-hour nurse call line.

Richland Care has facilitated care for over 18,500 participants and continues to actively work on providing care for the county’s population in need. In 2010, RC was selected as a participant of AccessHealth SC, a program of the South Carolina Hospital Association with funding from The Duke Endowment. Partners participated in strategic planning activities to improve the network’s efficiency and effectiveness.

Richland Care will continue implementation of the healthcare delivery system to improve access to care and improve health outcomes of the target population.  The program will also continue to be available to assist other communities in developing healthcare delivery systems for the low-income uninsured while maintaining collaborations with community and statewide partners to develop additional opportunities for future years.  The primary goals of the program are to improve health outcomes and the overall health status of its participants.
